Financial Assistance Program

At SmileKeepers, an important part of our mission is to provide quality dental care to the diverse communities that we serve. We recognize that many people are concerned with the financial challenges of their dental care. The SmileKeepers Financial Assistance Program offers need-based support to patients who may not have the financial means to pay for all of their dental expenses.

Effective June 1, 2019, certain of our SmileKeepers offices are offering a “sliding scale discount program” for eligible uninsured, underinsured, and/or low income patients, based on household and family size, and indexed to the Federal Poverty Level (“FPL”) guidelines in order to ensure fairness in the administration of these discounts.

To be considered for participation, please complete the Financial Assistance Program application. Proof of household income is required. SmileKeepers will evaluate your application along with all required documentation to determine your eligibility for the Program and will treat all information submitted confidentially. Eligibility will be re-evaluated at least annually and at any time there is a change in one or more of the application criteria. SmileKeepers reserves the right to modify the criteria considered for eligibility without notice.

Once assessed, qualified applicants who are within the income guidelines will receive a discount based on the sliding fee schedule. The sliding fee schedule applies only to basic services covered by the Oregon Health Plan. Cosmetic and elective services are not covered. There may also be additional charges for lab services. SmileKeepers will offer sliding fee scale discounts on primary care services for qualifying patients.

Sliding fee scale discounts apply only to services provided directly by SmileKeepers. Fees for labs, supplies, equipment and services not provided by SmileKeepers are determined by the entity providing them. Sliding fee scale discounts will be reviewed annually, and SmileKeepers reserves the right to withdraw, suspend and/or modify the Program at any time. Previously-approved treatment plans will continue to be honored through completion.

Primary care services for purposes of this Program are defined as those dental services covered by the Oregon Health Plan.

Qualifying patients are those with all incomes at or below 200% of the Federal Poverty Guidelines based upon total gross household income and the number of persons residing in the household. A nominal charge will be established for patients at or below 100% of the Federal Poverty Guidelines.

Patients must complete an application annually and furnish proof of income. If patients do not have proof of income they must provide a statement describing how they are supporting themselves.

Under no circumstances will SmileKeepers waive or discount amounts that do not meet the requirements outlined in this policy or the Program. This Program is not to attract or retain patients, but to assist qualifying patients who cannot afford primary care services.
